Texas Licensed ED RN Case Manager
Job Summary
To support optimal transitions of care, the part-time Texas Licensed ED RN Case Manager will monitor real-time 'Admit, Discharge, Transfer' notifications, provide proactive patient outreach, and coordinate discharge follow-up care remotely from Pearland, TX.
Key responsibilities
- Monitor ADT notifications to facilitate patient and provider outreach
- Coordinate discharge planning and follow-up care in collaboration with healthcare teams
- Utilize clinical guidelines to ensure continuity of care for patients transitioning to the next site of care
Required qualifications
- Undergraduate degree in Nursing
- Current, unrestricted Multistate Registered Nurse (RN) license
- 4+ years of diverse clinical experience with acutely ill patients
- Knowledge of utilization management and quality improvement
- Experience in discharge planning
