Job Summary

To support patient care coordination, the part-time Texas Licensed Home Health Coordinator will manage new referrals, document workflow tasks in the electronic medical record, and collaborate with healthcare professionals while working remotely.

Key responsibilities

• Receive and document new referrals in a timely manner
• Complete assigned workflow tasks in the electronic medical record
• Coordinate care with nurses, social workers, and caregivers

Required qualifications

• Graduate of an accredited school of Vocational/Practical nursing preferred; high school diploma or equivalent required for non-clinical candidates
• One year of experience as a Licensed Vocational/Practical Nurse, preferably in homecare
• Current driver's license
