Job Summary
To support patient care coordination, the part-time Texas Licensed Home Health Coordinator will manage new referrals, document workflow tasks in the electronic medical record, and collaborate with healthcare professionals while working remotely.
Key responsibilities
- Receive and document new referrals in a timely manner
- Complete assigned workflow tasks in the electronic medical record
- Coordinate care with nurses, social workers, and caregivers
Required qualifications
- Graduate of an accredited school of Vocational/Practical nursing preferred; high school diploma or equivalent required for non-clinical candidates
- One year of experience as a Licensed Vocational/Practical Nurse, preferably in homecare
- Current driver's license
