Job Summary
To support a centralized clinical call center, the full-time Texas Licensed LVN/LPN will manage a high volume of inbound and outbound patient interactions, triage alerts, and provide timely health interventions while working remotely.
Key Responsibilities
- Manage inbound and outbound telephonic patient interactions, conducting 70 to 90 calls per day
- Monitor and respond to Remote Patient Monitoring (RPM) alerts in real time, escalating concerns as necessary
- Collaborate with providers to coordinate effective patient care and perform monthly wellness assessments
Required Qualifications
- Active compact LVN/LPN license required
- Proficient with computers, EMRs, and telehealth tools
- Demonstrated AI fluency, actively using tools like Claude or ChatGPT in daily work
- Ability to work in a fast-paced, high-volume call center environment
- Comfort with performance-based metrics and accountability to targets
