Job Summary
Focused on managing healthcare costs for expectant mothers, the full-time Texas Licensed Maternity Case Manager will assess health statuses, coordinate care, and develop treatment plans while working remotely.
Key responsibilities
- Assess member health status and coordinate care, including inpatient discharge planning and outpatient management
- Monitor health status and adjust care plans based on changing member needs and treatment effectiveness
- Develop and implement short and long-term case management goals with documented care plans, ensuring timely and accurate reporting
Required qualifications
- 5+ years of professional work experience in clinical nursing and patient assessment
- Related Bachelor's degree or equivalent work experience
- Graduate of an accredited school of nursing
- RN - Registered Nurse - State Licensure in good standing
- RN Compact State Licensure
