Job Summary
Empowering families for optimal wellness, the full-time Texas Licensed Nurse Case Manager will work remotely to assess member needs, develop individualized care plans, and coordinate necessary medical services for high-risk populations in a telephonic managed care setting.
Key responsibilities
- Assess physical, functional, psychological, and environmental needs of members and develop tailored care plans
- Monitor and adjust care plans to ensure appropriate service provision and benefit adequacy
- Interface with healthcare providers to coordinate care activities and provide education to members about their treatment plans
Required qualifications
- Current RN licensure in Texas or Compact licensure
- Three or more years of clinical experience in a Level III or above NICU
- CCM certification required within specified timeframe
- Experience in a Case Management role and familiarity with maternity management programs preferred
- Proficiency in data entry into databases or EHR systems
