Job Summary
A company is looking for an Advanced Practice Provider (Nurse Practitioner / Physician Assistant).
Key Responsibilities
- Conduct comprehensive wellness exams in-home and virtually
- Educate patients and families about healthcare programs and treatment options
- Coordinate care with primary care providers and specialists
Required Qualifications
- Active and unrestricted RN and NP/PA license in the state of practice
- Board certification (NP: ANCC/AANP; PA: NCCPA)
- Minimum 2 years of direct patient care experience, preferably in managed care or chronic disease management
- Bilingual (English/Spanish) required
- Proficiency with electronic medical records (EMR) and telehealth platforms
