Job Summary
A company is looking for an Advanced Practice Provider, Nurse Practitioner/Physician Assistant in Home Care.
Key Responsibilities
- Provide in-home care to patients within a designated territory
- Manage chronic care and medication options for patients
- Collaborate with healthcare teams to ensure quality patient outcomes
Required Qualifications
- 3+ years of clinical experience as a Nurse Practitioner/Physician Assistant in primary or home care
- Current TX license and Nurse Practitioner certification from ANCC or AANP
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Working knowledge of chronic care management
- Current DEA licensure / Prescriptive Authority in TX
