Job Summary
Providing leadership in chronic care management, the full-time Texas Licensed Nurse Practitioner Team Lead will assist a clinical manager with onboarding, conduct chart reviews, and deliver ongoing clinical education while working remotely.
Key responsibilities
- Assist with onboarding newly hired Advanced Practice Providers (APPRs) and provide ongoing clinical education
- Conduct chart reviews to ensure compliance with processes and participate in committees enhancing chronic care management
- Serve as a liaison with departments such as Human Resources and IT to ensure proper training and credentialing of staff
Required qualifications
- Active and unrestricted Nurse Practitioner license in Texas, California, or New Jersey
- Master of Science in Nursing (MSN) with National Board Certification (AANP or ANCC)
- Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) certification
- At least 2 years of Advanced Practice Provider experience, preferably in primary care or chronic care management
- Proficiency in Microsoft Office, PCC, and Teams
