Texas Licensed Outpatient Care Manager

Job Summary

Managing care coordination and education for enrolled members, the PRN Registered Nurse Outpatient Care Manager will work remotely to support care management and disease management programs, ensuring effective communication and collaboration with healthcare teams.

Key responsibilities

• Effectively manage a caseload of enrolled patients through assessments and care plan development
• Provide education and support to members to achieve positive health outcomes and satisfaction
• Collaborate with interdisciplinary teams to enhance patient care and ensure compliance with regulations

Required qualifications

• Associates Degree in Nursing required; Bachelor's degree preferred
• 3 years of managed care, acute clinical care, or case management experience required
• RN licensure upon hire required
• CCM or ACM certification within 2 years of hire preferred
• Utilization management and pre-authorization experience preferred
