Job Summary

Leading clinical care delivery, the remote Texas Licensed Palliative Care Provider will coordinate services, support quality improvement initiatives, and collaborate with various teams to ensure high-quality supportive care for patients and their families.

Key responsibilities
  • Lead internal and joint case rounds to eliminate barriers to care and ensure appropriate care plan execution
  • Coordinate with Network Providers and Longitudinal Care Providers for optimal service delivery
  • Monitor and analyze network data to identify trends and implement necessary interventions
Required qualifications
  • Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies
  • Five years of clinical experience in palliative care, hospice, or with patients with serious illnesses
  • Active Registered Nurse or Physician Assistant license in Texas without board actions
  • Board certification as an advanced practice nurse by a nationally recognized certifying agency
  • Active Unrestricted DEA license
