Texas Licensed Patient Admission Coordinator LVN

Job Summary

To support home health operations, the full-time Texas Licensed Patient Admission Coordinator LVN will manage the timely acceptance or decline of patient referrals, ensuring effective communication with clients and referral sources while working onsite in Dallas, Texas.

Key responsibilities:

• Collaborates with referral sources, sales teams, and clients to gather necessary information for processing quality referrals
• Documents verbal orders and all referral-related information accurately and timely in the medical record
• Ensures prompt attention to referrals received through various platforms and reviews eligibility alerts for Medicare clients

Required qualifications:

• Current LVN license in the practicing state
• 3-7 years of experience in facility/physician relationships with knowledge of discharge processes
• Minimum of one year experience in home care, hospice, or personal care services
• Prior experience using Electronic Medical Records (EMR)
• Familiarity with various healthcare payors, including Medicare and Medicaid
