Job Summary

Supporting early care coordination, the remote Texas Licensed Patient Navigator will manage discharge planning, enhance member experiences, and collaborate with healthcare teams to ensure timely transitions from inpatient settings.

Key responsibilities
  • Facilitate timely and safe transitions of care for patients from inpatient settings to post-acute care
  • Identify and address barriers to discharge while ensuring regulatory compliance and effective communication with members and caregivers
  • Collaborate with Utilization Management and other healthcare providers to improve patient outcomes and reduce readmission risks
Required qualifications
  • Graduate of an accredited LVN program
  • Current & unrestricted LVN license in Texas or compact licensure
  • 5+ years of discharge planning experience
  • 3+ years of nursing experience in outpatient or acute care settings
  • Proficiency in care coordination and technology, including EMR systems
