Job Summary

A company is looking for a Pharmacy Clinical Program Lead - VBC.

Key Responsibilities

Develop and implement pharmacy-led initiatives and medication management strategies
Provide clinical consultation and guidance to multidisciplinary care teams on nephrology-related pharmacotherapy
Design training programs and educational resources for care teams and patients on medication safety and adherence

Required Qualifications

Doctor of Pharmacy (PharmD) degree from an accredited program
Active pharmacist license in specified states within 6 months of hire
Minimum 5 years of progressive pharmacy practice experience, including 1 year in a leadership role
Expertise in CKD and ESRD pharmacotherapy and value-based care models
Strong skills in formulary management and medication therapy management (MTM)
