Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN).

Key Responsibilities

• Manage patient census to close gaps in clinical and non-clinical care
• Identify and coordinate community resources for patient care
• Provide patient education on chronic condition management and perform medication management

Required Qualifications

• Active Multi-State/Compact License (LPN) (NLC) (LVN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote environment with high-speed Wi-Fi and a HIPAA-compliant workspace
• Experience in chronic care management
