Job Summary
Providing comprehensive virtual primary care, the full-time Texas Licensed Primary Care Nurse Practitioner will manage a patient panel through video visits, delivering assessment, diagnosis, treatment, and education while collaborating with a multidisciplinary team in a remote setting.
Key responsibilities:
- Deliver high-quality virtual primary care, including assessment, diagnosis, and patient education
- Manage a panel of patients, addressing acute, chronic, and preventive care needs
- Collaborate with the Care and Case Management team to develop effective care plans
Required qualifications:
- Minimum of four years of experience in an ambulatory or outpatient family medicine or internal medicine setting
- Active, unrestricted Nurse Practitioner and RN license in the state of residence; ability to obtain additional state licenses as needed
- Current board certification by a national certifying body through ANCC or AANP
- Experience independently managing a patient panel as the designated primary care provider
- Proficiency with telehealth technology and electronic health platforms
