Job Summary
To expand access to primary care, a full-time Texas Licensed Primary Care Physician will deliver comprehensive remote clinical care, including preventive services, sick visits, and chronic condition management, while managing a dedicated panel of patients.
Key responsibilities
- Provide high-quality preventative, acute, and chronic care in a fully-remote, virtual setting
- Manage a dedicated panel of patients and refer them to specialists or acute care services as needed
- Support population health initiatives and improve patient understanding of treatment plans post-discharge
Required qualifications
- Active Texas medical license with IMLC compact licensure
- Board-Certified in Family Medicine
- 3+ years of post-residency work experience, including 2+ years in an in-person primary care setting
- Experience with virtual visits and prior experience with EPIC EMR
- Strong interest in developing longitudinal relationships with patients and managing chronic care conditions
