Job Summary

A company is looking for an RN Plan of Care Coordinator - Remote.

Key Responsibilities

• Oversee the quality of home health patient Care Plans in collaboration with clinicians and coding coordinators
• Utilize various documentation to create detailed and individualized Plans of Care demonstrating home health eligibility
• Maintain case status information and notify leadership of problematic trends during case reviews

Required Qualifications

• Associate degree in nursing with current unencumbered licensure; Bachelor's degree preferred
• Minimum 1 year of healthcare experience; home health experience preferred
• 1 year of HomeCare Home Base (HCHB) experience strongly preferred
• Oasis Certification required for this role
