Job Summary
As a full-time Texas Licensed RN Case Manager working remotely, the candidate will create, implement, and monitor treatment plans to assist patients in achieving their healthcare goals while coordinating care with various healthcare providers and community resources.
Key responsibilities
- Assess member health status and care coordination needs, including inpatient review and discharge planning
- Develop and implement case management goals with documented care plans, monitoring health status and treatment plan effectiveness
- Identify and coordinate services, resources, and facilities to promote optimal value for clients and reimbursement sources
Required qualifications
- 5+ years of clinical nursing and patient assessment experience
- Related Bachelor's degree or equivalent work experience
- Graduate of an accredited school of nursing
- Active RN state licensure and Compact State Licensure in good standing
- Knowledge of medical terminology and cost containment practices in healthcare
