Job Summary
Providing comprehensive care coordination and case management services, the full-time Texas Licensed RN Case Manager will ensure seamless transitions of care, promote health stabilization, and reduce avoidable hospitalizations while working remotely.
Key responsibilities
- Manage high-risk and complex case management for members, including transitions of care
- Engage with members, families, healthcare providers, and multidisciplinary teams to create and implement individualized care plans
- Serve as a liaison between key stakeholders, including members, providers, and health plans
Required qualifications
- Associate degree in nursing
- Current Texas RN licensure
- Minimum of 2 years clinical experience
- Strong knowledge of case management principles and managed care
- Experience with care coordination and transition of care processes
