Texas Licensed Senior Care Manager

Job Summary

A company is looking for a Senior LTSS Service Care Manager (RN) in the Hidalgo SDA region of Texas.

Key Responsibilities

• Assess, plan, implement, and coordinate holistic care management for long-term care members
• Conduct screenings and risk assessments to identify member needs
• Develop and update care/service plans based on individual needs and clinical data

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Texas
• Associate's or Bachelor's degree in Nursing
• Minimum 1-2 years of experience in Case Management or Home Health
• Experience with Managed Care Organizations (MCO) is preferred
• Must pass a background check
