Job Summary
To support the Texas Child Health Access Through Telemedicine (TCHATT) program, the remote Senior Social Worker (LMSW/LCSW) will independently provide advanced social work services to assess and address the medical, psychiatric, and social needs of students, collaborating with multidisciplinary teams to ensure comprehensive care.
Key responsibilities:
- Conduct comprehensive assessments to interpret patients' social, emotional, environmental, and financial needs
- Develop treatment plans or refer cases to appropriate professionals to ensure patients receive necessary care
- Collaborate with healthcare providers to enhance patient experiences and evaluate individualized care plans
Required qualifications:
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Texas
- Master's Degree in Social Work from an accredited institution
- Minimum of 4 years of social work experience, including at least 1 year of evidence-based counseling
- Experience with trauma-specific treatment modalities preferred
- Knowledge of DSM-5 criteria
