Job Summary

A company is looking for a Social Worker (Remote) in Texas.

Key Responsibilities:

• Perform comprehensive evaluations and document findings in compliance with CMS standards
• Coordinate services for patients, utilizing community resources and ensuring compliance with health plan requirements
• Engage in interdisciplinary team meetings and maintain a patient case load with proper documentation

Required Qualifications:

• Bachelor of Social Work with a minimum of 3 years of relevant experience
• Current BLS certification or ability to obtain within 30 days of hire
• 3+ years of social work experience in a healthcare environment
• Proficiency in MS Office Suite and ability to manage multiple tasks simultaneously
• Must reside within the state of Texas
