Texas Licensed Social Worker

Job Summary

To support the Texas Child Health Access Through Telemedicine (TCHATT) program, a Texas Licensed Social Worker will provide remote care coordination for patients, manage their transitions through the TCHATT system, and educate healthcare providers on community resources and psychosocial factors impacting care.

Key responsibilities

• Independently provide social work services to connect patients and families with long-term care and community resources
• Plan treatment courses and manage patient transitions while coordinating care with healthcare professionals
• Educate staff and community providers on psychosocial factors affecting care needs and available resources

Required qualifications

• Bachelor's Degree in Social Work or a related field required
• Licensed Baccalaureate Social Worker (LBSW) by the State of Texas required; Licensed Master Social Worker (LMSW) preferred
• Minimum of 1 year of social work experience required; medical social work experience preferred
• Bilingual candidates are preferred
• Must reside in the Greater Houston area to work with the local patient population
