Texas Licensed Social Worker
Job Summary
Providing professional social work services, the remote Texas Licensed Social Worker will conduct comprehensive assessments, develop individualized care plans, and coordinate patient transitions within the TCHATT Program while assisting patients and families in accessing long-term care and community resources.
Key responsibilities
- Conduct comprehensive assessments to identify patients' social, emotional, environmental, and financial needs
- Collaborate with a multidisciplinary team to develop and monitor individualized care plans for patients
- Educate healthcare providers on community resources and psychosocial factors affecting patient care
Required qualifications
- Bachelor's Degree in Social Work or a related field required
- 1 year of social work experience required; medical social work experience preferred
- Licensed Baccalaureate Social Worker (LBSW) by the State of Texas required
- Licensed Master Social Worker (LMSW) by the State of Texas preferred
- Bilingual (English/Spanish) preferred
