Job Summary

A company is looking for a Tele-Health Nurse (LPN) to provide remote support to patients and healthcare providers.

Key Responsibilities:

• Provide telephonic and electronic support to stakeholders to enhance medication adherence
• Coordinate the collection of patient-specific clinical data for regulatory compliance
• Ensure understanding and compliance with regulatory and program requirements

Required Qualifications:

• Vocational or technical training is required
• Current Licensed Practical Nurse licensure is required
• Two or more years of experience in a healthcare setting
• Proficiency in data entry and Microsoft Suite software
• Ability to work in a fast-paced, changing healthcare environment
