Job Summary
Providing remote triage and care planning, the full-time Texas Licensed Tele-Triage LPN will assess health statuses, develop care plans, and manage symptom-based calls during service hours.
Key responsibilities
- Monitor assigned phone lines to ensure timely response to caller needs
- Triage symptom-based calls and provide recommendations according to approved protocols
- Document care-related recommendations and coordinate services with acute care settings
Required qualifications
- Degree/certification from an accredited school of Nursing
- At least one year of nursing experience
- Current licensure as a LPN in Texas, with a compact license in good standing
- Clinical experience with individuals with intellectual and developmental disabilities or related disorders
- One year of home health experience preferred
