Job Summary
Providing remote triage and care coordination, the full-time Texas Licensed Tele-Triage LPN will manage on-call responsibilities, assess health needs, and develop care plans for clients from Monday to Friday, 9 AM to 5 PM CST.
Key responsibilities
- Monitor assigned phone lines to ensure timely response to caller needs
- Triage symptom-based calls and provide recommendations according to established protocols
- Document care recommendations in real time and coordinate with local operations for follow-up actions
Required qualifications
- Degree/certification from an accredited school of Nursing
- Current licensure as a LPN in Texas, with a compact license in good standing
- At least one year of nursing experience, preferably in home health
- Clinical experience with individuals with intellectual and developmental disabilities
- One year of home health experience preferred
