Job Summary
Providing remote triage and care planning, the full-time Texas Licensed Tele-Triage LPN will assess health statuses, develop nursing care plans, and manage on-call responsibilities for assigned populations, working Monday to Friday from 9 AM to 5 PM CST.
Key responsibilities
- Monitor assigned phone lines and triage symptom-based calls according to protocols
- Document care recommendations in real-time and coordinate with local operations and acute care settings
- Develop and update care plans, ensuring compliance with state and payer requirements
Required qualifications
- Degree/certification from an accredited school of Nursing
- Current licensure as a LPN in Texas, with a compact license in good standing
- At least one year of nursing experience, preferably in home health
- Clinical experience with individuals with intellectual and developmental disabilities or related disorders
- One year of home health experience preferred
