Job Summary

Key Responsibilities

• Provide telephonic patient care and support, developing and evaluating comprehensive care plans
• Educate patients on medication adherence and disease management through outbound calls
• Collaborate with healthcare professionals to ensure continuity of care for defined patient populations

Required Qualifications

• Current unrestricted Registered Nurse license in Texas with multi-state/compact privileges
• Must reside in Texas
• Minimum of 3 years of clinical RN experience
• Proficient in Microsoft Office applications, including Word, Excel, and Outlook
• RN Diploma, Associate's, or Bachelor's degree in Nursing
