Job Summary

- Provide telephonic patient care, developing and evaluating comprehensive care plans for various disease states
- Educate patients on medication adherence and disease management through outbound calls
- Collaborate with healthcare professionals to enhance patient experience and continuity of care
Required Qualifications, Training, and Education
- Must be a licensed Registered Nurse (RN) in Texas
- Experience in telehealth or telephonic nursing preferred
- Strong clinical expertise in managing multiple disease states
- Advanced computer skills for patient interaction and documentation
- Ability to work Monday-Friday from 10:30 am to 7:00 pm (CST)
