Key Responsibilities

• Provide telephonic patient care by developing, implementing, and evaluating comprehensive care plans
• Educate patients on medication adherence and disease management during outbound calls
• Collaborate with healthcare professionals to enhance patient experience and continuity of care

Required Qualifications

• Registered Nurse with an unrestricted current compact license in Texas and ability to be licensed in multiple states
• Must reside in Texas
• 3+ years of clinical RN experience
• Experience using Microsoft Office, including Word, Excel, and Outlook
• RN Diploma, Associate's or Bachelor's in Nursing
