Job Summary

A company is looking for a Transition Specialist to manage the coordination of member transitions in the community.

Key Responsibilities

• Support care coordination teams and develop effective transition plans for members
• Assist with member transitions based on care service enrollment and ensure authorizations are honored
• Track transition metrics and develop educational materials for members and care coordination staff

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Behavioral health experience strongly preferred
• Equivalent experience may be considered in lieu of degree
• Knowledge of care coordination and community health services is beneficial
