Job Summary
To support a growing Transitions of Care program, the full-time Bilingual (Spanish/English) Transitions of Care Advanced Practice Provider will provide virtual clinical care post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team to prevent patient readmissions.
Key responsibilities
- Perform initial and acute symptom TOC visits, conducting comprehensive assessments and managing patient care
- Participate in interdisciplinary TOC rounds to discuss high-risk patients and enhance team communication
- Engage in quality improvement projects and provide clinical education to the team
Required qualifications
- At least 3 years of experience in a hospital, post-hospital, or primary care setting
- Active RN and APRN licenses or Physician Assistant license in good standing
- Ability to work full-time, including one weekend shift per month
- Active BLS certification at the time of hire
- Willingness to obtain licensure in required states within 90 days of hire
