Job Summary
Collaborating with referral sources and agency staff, the full-time remote Texas Patient Admissions Coordinator will manage the referral process, ensuring timely communication and documentation to achieve optimal patient satisfaction and outcomes.
Key responsibilities
- Maintains effective communication with referral sources, sales teams, and clients to collect necessary information for processing referrals
- Documents referral-related information and actions in the medical record accurately and within established time frames
- Ensures timely attention to referrals received through various platforms and follows up with clients and caregivers to confirm or collect additional information
Required qualifications
- High School diploma or GED
- Minimum of one year experience in home care, hospice, or personal care services
- Prior experience with Electronic Medical Records (EMR), preferably HCHB
- Experience dealing with various healthcare payors, including Medicare, Medicaid, and commercial payers, preferred
