Job Summary

A company is looking for a Therapy Continuity Coordinator who will engage patients in therapy and support their continuity journey.

Key Responsibilities

• Coordinate follow-up and documentation for patients transitioning to long-term resupply
• Execute timely resupply outreach and follow structured retention workflows
• Collaborate with clinical, billing, and operations teams to track engagement and reorder metrics

Required Qualifications

• 2+ years in healthcare coordination, DME, or care navigation
• Experience with PAP/CPAP resupply is a strong plus
• Ability to work autonomously in a fast-paced, remote environment
• Data-driven mindset with strong organizational skills
• Genuine commitment to improving patient experience and therapy adherence
