Therapy Continuity Coordinator

Job Summary

To support patient engagement in therapy, the part-time Therapy Continuity Coordinator will actively manage the continuity journey for patients enrolled in the PAP treatment program, ensuring a seamless resupply experience while working remotely.

Key responsibilities

• Coordinate follow-up and documentation during the transition from treatment setup to long-term resupply
• Execute timely resupply outreach and maintain accurate reorder tracking
• Collaborate with clinical, billing, and operations teams to monitor patient engagement and adherence

Required qualifications

• 2+ years of experience in healthcare coordination, DME, or care navigation
• Experience with PAP/CPAP resupply is a strong plus
• Strong patient communication skills with a high level of empathy
• Ability to work autonomously in a fast-paced, remote environment
• Data-driven mindset with proficiency in tracking tasks and following structured workflows
