Job Summary

A company is looking for an Transition Care Manager RN.

Key Responsibilities:

Identify and outreach to newly discharged patients within 48 hours
Complete comprehensive medication reconciliation and ensure patient understanding of discharge instructions
Collaborate with care team to develop and adjust individualized care plans

Required Qualifications:

BSN or MSN degree from accredited school of nursing
Current RN license in the state of practice
Minimum of 5 years' experience in clinical nursing
Current BLS certification
Basic computer skills and proficiency in MS Word and Outlook
