Transition Lead

Job Summary

To oversee the critical transition of a live public health system, the full-time remote Transition Lead will plan and execute a detailed 30-day transition-in process, ensuring no service degradation while managing knowledge transfer and operational readiness.

Key responsibilities

• Plan and execute the transition-in process, coordinating tasks across a two-sprint schedule and ensuring full service responsibility by day 30
• Institutionalize knowledge transfer by developing playbooks and documentation, while confirming system ownership transfer from the outgoing contractor
• Maintain transition-out readiness throughout performance, ensuring documentation is complete and accessible, and conducting regular readiness assessments

Required qualifications

• Bachelor's degree and at least 10 years of relevant delivery or program management experience
• Direct experience executing a transition-in from an incumbent contractor on a federal program
• Working fluency with agile delivery methodologies
• Demonstrated experience running knowledge-transfer programs with measurable completion criteria
• Ability to obtain a federal background investigation and PIV credential
