Job Summary
Guiding business stakeholders through the full transformation lifecycle, the full-time Transition Manager will serve as the primary client-facing leader responsible for overseeing opportunity identification, transition, stabilization, and optimization while working remotely.
Key responsibilities
- Provide end-to-end oversight across Discern, Design, Build, Transition, and Optimization phases while ensuring alignment with business leaders
- Coordinate cross-functional teams to execute transformation initiatives and manage risks, issues, and dependencies impacting client outcomes
- Monitor post-go-live performance and ensure successful handoff to steady-state operations, tracking progress and communicating outcomes to stakeholders
Required qualifications
- Bachelor's degree in business administration, Healthcare Administration, Operations, Education, or a related field
- 10-15+ years of progressive experience in transformation, operations, program management, or shared services environments
- Preferred certifications include Project Management Professional (PMP) and Prosci Change Management Certification
- Experience leading cross-functional teams without direct authority
- Understanding of change management and organizational readiness
