Transition RN Care Manager

Job Summary

A company is looking for a RN Care Manager.

Key Responsibilities:

• Identify and outreach to newly discharged patients within 48 hours
• Complete comprehensive medication reconciliation and ensure patient understanding of discharge instructions and newly prescribed medications
• Collaborate with care team to develop and adjust individualized care plans, provide education and support, and assess patient conditions

Required Qualifications:

• BSN or MSN degree from accredited school of nursing
• Current Registered Nurse (RN) license in the state of practice
• Minimum of 5 years' experience in clinical nursing
• Current BLS certification
• Basic computer skills and proficiency in MS Word and Outlook
