Transition Specialist

Job Summary

A company is looking for a Transition Specialist to manage the coordination of member transitions from inpatient hospitalizations.

Key Responsibilities

• Support care coordination teams and providers in developing effective transition plans for members
• Assist with transitions for members based on enrollment or care service needs
• Track transition metrics and maintain documentation to identify trends and improve processes

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience in service coordination, case management, or similar fields
• Experience in discharge planning or transition of care preferred
• Knowledge of community advocacy and care coordination processes
• Ability to develop educational materials and training programs for care coordination staff
