Transition Specialist

Job Summary

A company is looking for a Transition Specialist to support the coordination of member transitions in the community.

Key Responsibilities

• Support care coordination teams and providers in developing effective transition plans for members
• Assist with member transitions based on enrollment or care service needs
• Track and maintain transition metrics to identify trends and improve processes

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience or equivalent experience
• Experience in care coordination or health services preferred
• Knowledge of community resources and healthcare services
• Ability to work in a hybrid role with occasional travel
