Transition Specialist

Job Summary

A company is looking for a Transition Specialist to support the coordination of member transitions in the community.

Key Responsibilities

• Support care coordination teams to develop effective transition plans for members
• Assist with member transitions based on enrollment or care service changes
• Track and maintain transition metrics and document all transition of care information

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Equivalent experience may be considered in lieu of degree
