Transition Specialist

Job Summary

A company is looking for a Transition Specialist to support patients transitioning from acute care to post-acute services.

Key Responsibilities

• Support patients in transitioning from acute settings to home or post-acute care facilities
• Collaborate with case managers and providers to determine appropriate post-acute care services
• Arrange and document post-acute care and transportation needs, ensuring effective communication with patients and families

Required Qualifications

• Education: High School diploma or GED
• Experience: 2 years in health care and/or customer service
• Proficiency in Microsoft software and solid computer skills
• Demonstrated ability to work independently and prioritize tasks effectively
• Familiarity with medical terminology is preferred
