Job Summary
A company is looking for a Transitional Service Liaison-LPN/RN/Experienced CMA.
Key Responsibilities:
- Implement Transitional Care Program within Hospitals
- Coordinate care across the continuum in collaboration with internal case management and discharge planning teams
- Educate hospital team on specialty programs and collaborate on performance improvement opportunities
Qualifications, Training, and Education:
- Current LPN or RN license in good standing, or a seasoned CMA with business development skills
- Graduate of an accredited nursing program
- 10+ years hospital experience, with at least 5+ years in hospital care management
- Proficiency in collaborative tools such as Word, Excel, Powerpoint, and Access
- Experience in handling multiple projects and deliverables within deadlines
