Transitions of Care Coordinator

Job Summary

A company is looking for a Transitions of Care Coordinator.

Key Responsibilities

• Call members post-discharge to check on their well-being and resource needs
• Coordinate with healthcare providers and agencies to ensure members receive necessary support
• Assist in scheduling post-discharge visits and support the TOC clinical team with operational tasks

Required Qualifications

• Medical Assistant Certification preferred; prior medical office experience is highly desirable
• Minimum of 2 years of experience in a healthcare setting, managed care, or community health
• Must be available to work Monday-Friday from 10am-6:30pm EST
• Fully bilingual in English and Spanish preferred
