Job Summary

A company is looking for a Bilingual Medical Assistant for remote Transitions of Care position.

Key Responsibilities:

Provide support for Transitions of Care program through telephonic outreach and coordination
Advocate for patients and caregivers during transition home from a facility
Ensure data quality and accuracy, along with providing administrative support

Required Qualifications:

Medical Assistant certification
Fluency in English and Spanish (writing, reading, speaking)
2+ years of experience in urgent care/acute care setting
Experience with symptomatic members and post-facility/hospital care
Remote work experience and ability to work Monday - Friday, 9:00 am - 6:00 pm EST
