Transitions of Care Nurse Practitioner

Job Summary

To support the development of a comprehensive Transitions of Care program, the full-time Advanced Practice Provider will provide virtual clinical care post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team to prevent patient readmissions.

Key responsibilities

• Perform initial and acute symptom TOC visits, including comprehensive assessments and medication reconciliations
• Participate in interdisciplinary rounds to discuss high-risk patients and enhance team communication
• Engage in quality improvement projects and adapt to evolving healthcare needs and practices

Required qualifications

• Minimum of 3 years of experience in hospital, post-hospital, or primary care settings
• Active RN and APRN or Physician Assistant licenses in good standing
• Ability to work full-time, including one weekend shift per month
• Active BLS certification at the time of hire
• Willingness to obtain licensure in required states within 90 days of hire
