Transitions of Care RN

Job Summary

A company is looking for a Transitions of Care RN.

Key Responsibilities:

• Provide transitional case management for senior patients to improve clinical outcomes and reduce readmissions
• Coordinate care for patients post-discharge from acute care facilities
• Act as a liaison between healthcare providers and patients' primary care teams during transitions

Required Qualifications:

• Licensed Registered Nurse with the ability to obtain licensure in multiple states
• 5+ years of RN experience with at least 1 year in care coordination/case management
• Demonstrated experience in complex care settings, senior health, or case management
• Advanced knowledge of utilization management/care management principles
• Understanding of Mac iOS, Google suite
