Job Summary
To enhance patient navigation through the healthcare system, the full-time Transitions of Care RN Case Manager will provide telephonic, short-term care management for high-risk patients post-discharge, advocating for their needs and coordinating necessary resources remotely.
Key responsibilities
- Engage with newly discharged patients to assess their needs and develop personalized care plans
- Collaborate with healthcare providers to ensure coordinated care and smooth transitions during the post-discharge period
- Educate patients on chronic conditions and facilitate access to community resources for better health outcomes
Required qualifications
- Unrestricted Compact RN license in the United States, with the ability to secure additional state licenses
- 3-4 years of RN care management or telephonic case management experience
- Availability to work Monday-Friday from 8:30 AM to 5:00 PM MST/PST
- Experience in health insurance, particularly Medicare Advantage
- Strong analytical thinking skills to address individual patient needs effectively
